Our Privacy Notice is a document which describes how medical information about our patients may be used and disclosed, and how patients can exercise a number of new rights granted under the Health Insurance and Accountability Act (HIPAA).

We encourage you to review this Privacy Notice and become familiar with how your personal health information will be used and safeguarded, as well as rights regarding the protection of your personal data. The information in the Privacy Notice is effective April 14, 2003.
